The undersigned agrees to pay the following applicable program fee each month in 12 equal monthly installments:

Regular Program:  $125 (HST included) monthly

Tiny Tiger Program:  $65 (HST included) monthly

The first monthly installment being due on/or before the date of the member/students first lesson. With all subsequent installments being due on/or before the 1st day of each month.*

Fees that are not paid by the first of the month will result in the students inability to attend classes. Late payments will be subject to a late payment fee of $20.00 per occurrence.*

All payments can be made by cash or by e-transfer to silenttigerma.2022@gmail.com. Cash payments must be delivered in person in a sealed envelope labelled with the students name and date delivered marked clearly on the outside of the envelope. Please note that payments are to be exact amount only, as change will not be provided.*

At this time, Silent Tiger Martial Arts is not accepting Debit, Visa Debit, Cheque, Pre-authorized Debit or any credit card payments for memberships, uniforms, equipment, clothing, or gradings.*

At this time, Silent Tiger Martial Arts will not be issuing any receipts for memberships, uniforms, clothing, equipment or gradings.*

The undersigned agrees to receive and participate in, and the school agrees to teach a course of martial arts lessons consisting of a maximum of three lesson days per week to be taught during the next 12 months.*

Members are eligible to train three days per week. Any students wishing to participate in multiple classes on the same day must be approved by the head instructor Sensei Ryan Fice.*

If you wish to cancel this contract, you may do so by written notice to Silent Tiger Martial Arts. The notice must state that you no longer which to be bound by this contract and must be delivered in person or electronically to silenttigerma.2022@gmail.com.*

If you choose to terminate your contract any monies already paid to Silent Tiger Martial Arts for that month or subsequent months will not be returned. *

Cancellation will become effective immediately from the date the written notice is received.*

Silent Tiger Martial Arts reserves the right to alter days, hours of operation, course curriculum or location of offered classes if deemed necessary.*

The student and co-signed agree unconditionally to pay the above tuition without regard to the attendance made or classes missed on behalf of the member.*

Monthly membership fee does not include the cost for promotion/gradings. Cost for promotion/grading of Kyu Belts is $40.00. Cost for promotion/grading of Junior or Adult Black Belts is To Be Determined at the time an invitation to promote has been awarded.*

All members/students of Silent Tiger Martial Arts must supply their own protective equipment and weapons.*

Silent Tiger Martial Arts reserves the right to cancel any contract. *
